Question SCENARIO A male in his mid-sixties presented to his St Bryans… SCENARIOA male in his mid-sixties presented to his St Bryans Hospital emergency department (ED) with complaints of shortness of breath and chest pain. He had a history of chronic obstructive pulmonary disease (COPD) and cardiovascular disease (CVD).  The patient was admitted for exacerbation of COPD symptoms and atypical chest pain.  At various times during the admission process, the patient was noted to be a ‘falls risk’ but not fully recorded or assessed using a falls risk tool.  He fell twice while in the acute ward in the am shift.  He was then found later in the day on the floor near his bed, and was subsequently diagnosed with a fractured neck of femur.  This resulted in the patient having to undergo a surgical procedure to treat the fractured neck of femur.  The patient stated that he had used the call bell to request assistance getting to the bathroom but no- one responded. As a result, the patient attempted to get out of bed on his own and got caught in wires and cords that were attached to him, causing him to fall. On investigation of the incident, the team found that there were several safety issues that contributed to his fall. These included no bedside commode, no access to the bed alarm, no signage on the door or a wrist band. The number of falls on the ward has also increased over the recent months.  As part of the St Bryans Hospital Best Practice committee you are required to research the current nursing literature about nursing management and prevention of falls.  You will then present a report on your findings, and make recommendations for possible ‘nursing best practice standards or quality improvement strategies’ that would reduce the risk of falls for other patients in the facility. Please use the template and headings below for the creation of this report.   Dear Team Members,There has recently been some increases in falls within our hospital.  All members of the Best Practice committee are required to research current ‘nursing research data’ about the nursing management and prevention of falls.  You will then present a report on your findings, and make recommendations for possible ‘nursing best practice standards or quality improvement strategies’ that would reduce the risk of falls for other patients in our hospital. There are important issues that we need to resolve regarding the increased prevalence of falls, and achieve a team decision to reduce the occurrence of falls. The recommendations will be discussed by the committee, to then agree to best practice actions for implementation by the hospital. .                                                                                  Lauren Wilson (Chair)  Assessment Instrument: Questions and Observation Checklist Student Name:                                                                                                                    Student ID: Read the scenario on falls and answer the following questions related to the scenario. Explain the situation in this scenario and why research may be required to support and improve work practices, and your own nursing practice as an employee of this simulated workplace.  (1-2 typed lines approximately 20-50 words)  Review the different sources of information available about preventing and managing falls (eg nursing practice standards, journal publications, senior nurses or nurse specialists, falls management interest groups or working parties).  Identify the current trends for falls prevention and management described within these information sources.  ie describe current changes and actions that are occurring for preventing and managing falls described in these different sources.  Note a minimum of 3 trends.  (2 typed lines or multiple dots points of approximately 30-50 words) Evaluate these current trends about preventing and managing falls in nursing practice.  ie evaluate current changes and actions that are occurring for falls prevention and management.  (2 typed lines of approximately 30-50 words) Create a personal research objective for locating information about ‘preventing and managing falls’. (1-2 typed lines approximately 20-50 words)  Select and evaluate possible methods of gathering information on preventing and managing falls. You will need provide no less than three examples of methods you could potentially use (for each method provide one to two sentences 20-50 words)Method of gathering information on falls prevention and management Evaluation of this method for gathering information        Use a systematic approach to complete a literature search, and access nursing clinical research articles about falls prevention and management. Make notes of how you completed this search in the table below.List your search terms, the journal databases or other sources used and keywords, including Boolean phrases/terms.  You may use a variety of key terms or methods to refine your search.  At least two examples are required. Credible databases or sources used in your literature searchKeywords  used in the literature search        From this literature search, locate journal articles related to falls management and prevention. From these different articles, you will then need to select 1 journal article to use.  It must have been published within the last 10 years, nursing oriented and based on nursing research. Explain how you reviewed the different articles in your literature search, and then made your final decision about which journal article was the most relevant to your personal research objective.    (2-3 typed lines of approximately 50-70 words) Compile the information from your selected article in the table provided below to prioritise and organise your information and support your analysis.  This compiled information will then be presented to the best practice committee. Table to review and organise article informationAWhat is the title of your journal article? Provide APA reference   (Bring this article to the committee meeting and also attach this article to your final submission)BBriefly summarise and prioritise the findings in the research article.  Prioritise the important information based on your need to make recommendations about pressure injury prevention or management. CExplain how you decided the information and research in this journal article was the most relevant in meeting your personal research objectives and the requirement for this committee meeting. DWhat year is your research article?    Based on this year of publication, assess the currency of the information in your journal article.   EExplain how the author/s, type of journal, sample size/ type, and the type of research process used by the researcher makes this a strong article to guide your evidence based practice decisions. FExplain the reliability of the information in your research article.(ie is the information credible? is the data or decision supported by statistics to make their final decisions and recommendations? could you recreate this research? Is the sample relevant to the data?) GReview the final recommended actions in this journal article then evaluate and make a judgement about the feasibility (capable or possible) for implementing these actions to falls prevention and management within the workplace. HReview the final recommended actions in this journal article then evaluate and make a judgement about the benefits for implementing these actions to   prevention and manage falls within the workplace. IReview the final recommended actions, in this journal article then evaluate and make a judgement about any possible risks if you implemented these actions to prevent and manage falls within the workplace. JCompare and contrast the falls management clinical practice guidelines in the Blackboard shell to the recommended actions in the journal article.    State what is similar between the two:    State what is different between the two:    KReview all of the points you have already included in this table about the research article and create a final conclusion about what you would recommend to the practice committee. L Review all of your collected information and assess the different actions that could be implemented within the hospital to prevent and manage falls.  Use dot points to list the information that could be used M Based on the information you have reviewed, create three final recommendations to reduce the number of falls in the hospital? NIdentify if you have located sufficient information to make a decision or if further research is required, or if the information in the journal article requires further research and testing.  Provide a reason for your decision.  ODevelop actions and a plan to implement your final decisions and outcomes (ie how you will complete further research or how you will implement your recommendations) as a recommendation to the committee.  This can include a plan with suggestions of how to support the implementation of the research findings.Make notes of these agreed actions    Health Science Science Nursing CHCPOL MISC Share QuestionEmailCopy link Comments (0)
